Welcome to the Peru School District Nurse's page.  We are here to provide your student a healthy and safe learning environment. Below you will find resources and links to school health requirements, as well as printable forms. Do not hesitate to reach us if you have any further questions.

Preschool Requirements

Physical Exam (Parents/Guardians please complete the Health History portion, sign and date). Complete updated immunization Record signed and stamped by your healthcare provider.

Kindergarten Requirements

Second Grade Requirements

Dental Examinations must be performed by a licensed dentist and submitted prior to May 15th of the school year. Dental exams must be completed on the Illinois Form dated between November 2024-May 15th, 2025. 

Sixth Grade Requirements

As of the 24'-25' School Year - Cough Drops/Throat Lozenges brought to school will require a Med Auth Form

IDPH Hearing & Vision Screening Mandates Link

Students are routinely screened for vision in Early Childhood, K, 2, 8, and for hearing in Early Childhood, K, 1-3. Students in special education are screened for vision and hearing annually. Students can also be screened by teacher or parent request, if a child exhibits symptoms. This is only a screening and DOES NOT take the place of an examination by a physician. A more comprehensive vision exam can be obtained through an optometrist or ophthalmologist. 

Your child is not required to participate in the vision screening if you have submitted a report signed by an optometrist or opthalmologist indicating that your child has had an eye exam within the previous 12 months.
